The House Call Program is a supplementary service where CNPs (NPs for NY) go into a member’s home (or via telehealth) to administer an “in-home health assessment” and address open care gaps in support of Risk Adjustment and Stars initiatives. This service is coordinated with vendor partners Matrix Medical Network, Signify Health, and Optum.
Highmark encourages member/provider follow-up to review the outcome of the member’s health assessment, as well as any lab results.
- The member’s in-home health assessment and lab results are shared with the member and member’s PCP.
- Highmark continues to promote Annual Wellness Visits (AWVs) and other healthcare engagements with a PCP early and often.
These in-home health assessments are provided to Highmark Medicare Advantage members residing in AR, AZ, CO, DE, ID, MA, ME, MN, MO, MS, MT, NE, NH, NJ, NM, NV, NY (Northeastern), OH, PA, RI, SC, UT, VT, WA, WV, and WY.
